I fully understand and agree that by signing this Apostille Request Agreement, I am entering into an agreement with Mark D. Miller and MDM Notary Services and agreeing to the following terms: (1) All fees must be paid in full prior to the start of services.  (2) A $25 handling fee will be charged for any returned checks. (3) I agree to pay any and all legal and collection costs incurred by Mark D. Miller and MDM Notary Services in the process of collecting returned checks or unpaid fees. (4) Liability for any documents that are damaged or lost in transit is limited to $100 per document. Mark D. Miller and MDM Notary Services are not responsible for documents lost, misplaced, or improperly routed by any consular office, embassy, or U.S. local, state, or federal government. agency.
